What do I need to do to become a patient at SMCHC?
Call the medical clinics or dental clinic to schedule an appointment. Staff will inform you of the necessary paperwork you will need to bring with you.
What documents am I required to bring to my first visit as a new SMCHC patient?
Bring your insurance or medicare card or if applying for the slide program bring proof of income, paycheck stub or unemployment letter.
What is the cost per visit?
Patients with insurance pay the co-pay of their plan due the day of the office visit. Patients without insurance are served on a sliding fee with a minimum of $20.00 co-pay due the day of service.
What insurance plans do you accept?
We accept most insurances, Medicaid and Medicare.

Are you a free clinic?
No, SMCHC is a Federally Qualified Health Center (FQHC). Our provider staff are full-time board certified physicians, dentists, and nurse practitioners. Most insurance plans are accepted. Patients without insurance are served on a sliding fee based upon income and family size.
Do you accept walk-ins?
Yes but scheduled appointments are preferred.
Do you offer free prescription drugs?
For our patients who qualify, we offer Medication Assistance.
How long does it take to get a refill called in?
48-72 hours.
